(Registration Fee does not include Mandatory League Fundraiser)

EARLY BIRD SIGN-UPS

$25.00 off if paid in full before April 30th 2010

Registration cost increases $50.00 if signed up after 7/15/2010

Documentation needed:

1.) Original Birth Certificate 2.) Entire Year Report Card (must have 2.0 GPA)

3.) Physical on 2010 Pop Warner form 4.) 2" inch photo must be recent
